About Deoochform

Deoochform is a hosted form builder with a remote MCP server for creating forms, reading submissions, and making invitations. It serves people working with MCP-compatible assistants and combines visual form editing with natural-language creation; respondents submit through public links without signing in.

Key features

  1. 01Create and update forms
  2. 02Read form submissions
  3. 03Create and publish invitations
  4. 04Browse form and invitation templates
  5. 05Visual form editing
  6. 06Public forms without respondent sign-in

Use cases

  1. 01Build a form through an MCP client
  2. 02Review responses to a form
  3. 03Create and share an invitation
  4. 04Let respondents submit through a public link

MCP support

The hosted remote MCP server exposes tools for templates, forms, submissions, and invitations. It connects over streamable HTTP using OAuth; the v2 endpoint excludes payment fields.

Configuration

Remote servers need no local install. Pick your client, then copy the config or install in one click.

Endpointhttps://deoochform.com/api/mcp/v2
Copy install command
Claude CodeTerminal
Cursor.cursor/mcp.json
VS Code.vscode/mcp.json
Claude Desktopclaude_desktop_config.json
Windsurf~/.codeium/windsurf/mcp_config.json

Run commands in a terminal; paste JSON into the client’s MCP config and restart. Servers that need sign-in will prompt on first connect.

What this is

Deoochform is a form builder with a remote MCP server built in. You can drag fields around in the visual studio like any other form tool, or skip that entirely and ask your assistant to build the form for you, then read the responses back the same way.

Respondents never sign in. They open a public /f/<slug> link, fill it in, and submit.

This repository is the public home of the connector: the manifest, the tool reference, and the setup notes. The product itself is hosted, so there is nothing to install or run.

Endpoint

https://deoochform.com/api/mcp/v2

Remote, streamable HTTP, OAuth. There is no API key to copy or store.

Connect it

Every client is the same three steps: add a remote or custom MCP connector, paste the URL, sign in when the browser opens.

ClientWhere
ClaudeSettings → Connectors → Add custom connector → paste the URL
ChatGPTSearch deooch in the app directory, or Settings → Connectors → Create custom connector
CursorSettings → MCP → Add new MCP server → type http → paste the URL
Windsurf, Zed, othersAnywhere that accepts a remote MCP server URL

A browser tab opens, you approve the connection, and you are done. The connection acts as you, with your role and your plan.

Tools

Forms

create_formCreate a new form and return its public link
update_formUpdate a form's title, description, fields, or status
get_formGet a form's full definition
list_formsList forms in this workspace

list_submissionsList submissions for a form
get_submissionGet a single submission by id

Invitations

list_invitation_templatesBrowse invitation designs
create_invitationCreate an invitation
publish_invitationPublish one and return its public link

Ten tools, and the list is the permission model: the server cannot do anything these do not expose. There is no delete tool, no billing access, and no reach into workspace members or other workspaces. update_form can set a form to closed, which stops new responses without destroying anything.

See docs/tools.md for parameters and return shapes.

Try it

Once connected, ask in plain language:

Create a customer feedback form with a star rating, a "what could we
do better" box, and an optional email field. Give me the link.
How many people filled in the feedback form this week, and what are
they complaining about?

The assistant calls create_form, you get a shareable link back, and list_submissions reads the answers when they arrive.

What the product does

  • 13 field types plus sections, validation, and required fields
  • Conditional logic on every plan, including free
  • Payment fields through Stripe: product, subscription and donation
  • Conversational layout, one question per screen, as a per form setting
  • Answer piping, so a later question can quote an earlier answer
  • Quiz scoring with an answer key and per question points
  • Notifications by email, Slack, or Google Sheets sync
  • Export to CSV and Excel

Authentication

Browser based OAuth 2.1 with PKCE. No token to paste, nothing stored in a config file. Dynamic client registration is supported, so most clients need nothing but the URL.

Discovery follows the spec, so a client that has never seen the server can find its way in unaided:

/.well-known/oauth-protected-resource/api/mcp/v2 → who authorizes this
/.well-known/oauth-authorization-server → the OAuth endpoints

Details, including the consent step and why PKCE alone is not enough, are in docs/authentication.md.

Two endpoints

URLPaymentsUse
/api/mcpEnabledThe original connector. Existing clients stay pointed here.
/api/mcp/v2DisabledListed in public directories. Cannot see or create payment fields, so nothing built through it can take money.

Capability is settled per URL rather than per request, because an app directory registers its OAuth client against a base URL and then freezes it. If you are adding the connector today, use /api/mcp/v2.

New to MCP?

From what the protocol is to installing your first server, these answers cover most first questions.

An open protocol that defines how AI applications connect to external tools and data in a uniform way. Clients talk to the model; MCP servers provide tools, resources and prompt templates. They exchange standard messages, so one server can be reused by many clients.
