About full-review
Full Review is a skill for evidence-based reviews of code changes and requirements documents. It helps reviewers and delivery workflows define review scope, verify findings, and produce a verdict without expanding beyond the approved plan.
Key features
- 01Scope contract for review depth
- 02Evidence-backed findings and verification
- 03Risk signals for code review scope
- 04Specialist routing under approved plans
- 05Structured verdicts for code and documents
Use cases
- 01Review a bounded code change
- 02Assess security-sensitive changes
- 03Check a plan or requirements document
- 04Recheck findings against updated evidence

From the project READMEFull Review
Choose the branch before selecting a model. Coordinator-only review and deterministic evidence work proceed within invocation authority. Apply routine review lenses in the assigned reviewer context. For a worker dispatch, use shared/references/model-preview.md and its one approved route snapshot. Nested skills, runners, and rechecks consume that snapshot. They do not select another route or worker.
For a worker dispatch, model IDs, effort support, and task defaults come only
from shared/model-routing.json. Preserve the exact saved route during
dispatch, retry, and resume.
Review the risks that a focused implementation pass cannot see. The result is a precise, evidence-backed verdict. The next consumer is the author or delivery workflow. A review is complete when its scope, evidence, findings, verification limits, and verdict are explicit.
Review plan
The review plan is a scope contract. Do not claim coverage outside it. It states scope, risk, seats, effort, and verification. In a delivery workflow, derive it from the user-approved routing-plan JSON. That JSON is the exact executable source for seat, runner, model, effort, mode, and unavailable action.
| Scope | Use when | Minimum review shape |
|---|---|---|
focused | One bounded change with local risk | One review seat and relevant deterministic checks. |
seam | Integrated tasks, shared contracts, or cross-boundary behavior | Approved route or routes that cover logic and state plus root-cause precision. Add a second route only when the plan names it. |
deep | Explicit deep request, security-sensitive boundary, migration, public contract, or high residual risk | The seam shape plus selected security, data, compatibility, or structural specialists. |
Do not launch a blanket panel. Seat names, model choice, runner, and effort come only from the approved plan and the model roster. Use the supplied or inherited review mode and scope. Otherwise use the narrowest safe scope the evidence supports. Ask only when the choice changes scope, cost, permitted effects, or a material unresolved decision. Silence is not approval. Do not dispatch an external route until its plan is approved. A direct request for a deep review authorizes the deep scope, not an unplanned route dispatch.
For a worker dispatch without a matching snapshot, model-preview.md uses
task-shaped-model-routing.md,
model-roster.md, and
host-model-execution.md
to form the one selection. A matching snapshot is consumed unchanged. The review
plan names the resulting route; this skill does not replace it with a convenient
runner or the coordinator model.
Workflow
- Establish the target: pull request, commit range, local diff, task file set, or document. An empty diff is an outcome, not a failure. Select the output contract now. A supplied shared review snapshot uses the selected sections of
shared/references/reviewer-response.md, or the same sections embedded in the brief. Standalone code usesreferences/review_output_schema.json; a standalone document usesreferences/doc-findings-schema.json. Load only that schema. - Read the active project conventions already in context and the local contracts that affect the changed paths. Collect the diff, changed files, relevant surrounding code, and existing review comments. Existing comments are candidates, not facts.
- Run
scripts/review_scope.pyfrom this skill's directory when reviewing code. Use its risk signals to confirm or narrow the plan; do not widen a focused review merely because a helper is available. - Load only the reference that matches a selected concern:
references/bug_finders.md,references/panel_roles.md,references/conditional_specialists.md, orreferences/structural_quality_review.md. For a requirements document, use only the relevant persona fromreferences/doc-personas/. Readreferences/review-dispatch.mdonly before an actual worker dispatch. - Routine concern lenses are read only in the assigned reviewer context. Dispatch an independent specialist only when a material risk or unresolved question requires it and the approved snapshot names its route. Give every dispatched seat the same scoped context, conventions, and output contract. Preserve seat fidelity: an unavailable seat is recorded as unavailable, never replaced silently. Use the exact native model and effort in an isolated persistent subagent or supported task context when the host provides it. Use a runner only for a foreign or unsupported route. Keep each reviewer context for later rechecks, and keep it separate from the implementer and other independent reviewers. Run independent seats concurrently when the host supports it. A final code review remains independent from the implementer.
- Verify runtime, security, correctness, compatibility, reliability, and performance findings when execution is possible. Evidence-check structural findings against the changed code. Mark an unverified claim as unverified or lower confidence; do not turn it into a blocker by assertion.
- For code, read
references/filtering_pipeline.md. Use the output contract selected at entry. Deduplicate, retain only evidence-backed findings with a location, then apply the active confidence threshold. Keep every confirmed finding durably; the cap limits only the human summary. Precision over volume: do not report cosmetic preference, broad refactor wishes, or pre-existing issues outside scope. - Return the verdict. The default is report-only. When the user or caller has explicitly authorized fixes, record
apply_fixes: trueand apply them within that scope; no second approval or exact user syntax is needed. Review the changed paths before delivery.
Security and documents
When security_focus=true, check the implementation against the recorded security decisions and include only the security and data specialists that the plan names. Do not drop a security finding to meet a comment cap.
For a plan or requirements document, review feasibility, scope, coherence, and decision gaps instead of a code diff. Keep the same scope contract and evidence bar.
Output
When the caller supplies a snapshot from shared/references/review-evidence.md,
use that shared contract for the complete final JSON response. Use the brief's
embedded sections when supplied. Otherwise read the introduction, Initial response, Evidence packet substitution, and Completion in
shared/references/reviewer-response.md. Add Recheck for a source recheck and
also Prose correction for an addendum. Add Batch endorsement only for a
declared batch, which requires a full response. A changed acceptance contract
also requires a full response. These sections replace the standalone schemas
and report template. Keep any human report separate from the recorded JSON.
Use the incremental response protocol from that contract for focused rechecks or prose addenda. The expanded record must account for every snapshot path and retain earlier finding IDs,
and reference captured check and browser evidence. Map CRITICAL, HIGH, MEDIUM,
and LOW to P0, P1, P2, and P3. Every confirmed MEDIUM maps to an open P2 until evidence closes it; a presentation cap cannot remove it. The coordinator records the actual response and
runs the verifier; execution success is not approval.
For standalone code without a shared snapshot, return a human report and JSON that matches references/review_output_schema.json. For a standalone requirements document, use references/doc-findings-schema.json instead. Only these standalone modes use references/review_report_template.md for the report.
In the standalone schema, every new code finding includes a stable ID, status (confirmed, unverified, or refuted), severity, confidence, category, location, evidence, smallest useful fix, and verification. Rechecks preserve IDs and disposition evidence. A document finding uses its section and quoted evidence. End a standalone code report with:
Bugs found: N | Verified: X | Refuted: Y | Verdict: APPROVE|COMMENT|REQUEST_CHANGES
State the selected scope, planned and unavailable seats, verification performed, and any coverage limit in the separate human report. For standalone verdicts, REQUEST_CHANGES requires a remaining critical or high finding; COMMENT requires a meaningful medium finding; otherwise use APPROVE.
