About handoff
handoff is a skill that creates a document summarizing a conversation so a new agent can continue the work. It follows a specified structure, saves the file in a temporary directory, and omits sensitive information and details already captured elsewhere.

Details
From the project README撰写一份交接文档，总结当前对话内容，使全新的 agent 能够无缝继续工作。
输出位置
保存到 $TMPDIR/handoff-YYYY-MM-DD-HHMM.md（用实际时间戳替换）。生成后告知用户完整路径。
文档结构
按以下结构组织交接文档：
- 背景与目标 — 一句话概述当前任务
- 已完成工作 — 带路径/URL 引用
- 当前状态 — 进展到哪一步、卡在哪里
- 待办事项 — 下一个 agent 需要做什么
- 推荐技能 — 建议调用的 skill 列表
- 关键上下文 — 环境、分支、配置等必要信息
规则
- 不要重复已存在于其他产物（规格文档、计划、ADR、issue、commit、diff）中的内容，改为通过路径或 URL 引用。
- 脱敏处理：删除 API key、密码、个人身份信息等敏感内容。
- 如果用户传入了参数，将其视为下一次会话的重点方向，据此调整文档内容。
- 如果当前对话极短（少于 3 轮实质交流），直接告知用户无需生成交接文档，除非用户坚持。
